When people think about supplements for cold sores, lysine is often the first nutrient that comes to mind.

But another nutrient has been studied for its potential relationship with herpes simplex virus (HSV): zinc.

Zinc is an essential trace mineral involved in hundreds of biological processes throughout the body. It is particularly important for maintaining normal immune function, supporting skin integrity, and regulating the activity of numerous enzymes and proteins.¹

What makes zinc particularly interesting in HSV research, however, is that its potential role may extend beyond general immune support.

Laboratory studies have investigated whether zinc ions can interact directly with HSV and interfere with different stages of viral infection. Human research has also explored topical zinc preparations, systemic zinc supplementation, and the relationship between zinc status and recurrent herpes labialis.¹⁻⁶

So what does the evidence actually tell us?

Why Is Zinc Important for Immune Function?

Zinc is essential for the development and function of the immune system.

It contributes to both innate immunity, the body’s immediate defense against pathogens, and adaptive immunity, the more targeted immune response that develops following exposure to a pathogen.¹

Adequate zinc status helps support the function of immune cells involved in antiviral defense, including:

  • Natural killer cells
  • T cells
  • Macrophages
  • Neutrophils
  • Other immune cells involved in recognizing and responding to infection¹

Zinc also plays a role in maintaining the physical barriers of the skin and mucous membranes, which form an important first line of defense against pathogens.

This is particularly relevant to HSV because the virus establishes lifelong latency after the initial infection. Between outbreaks, HSV remains dormant within sensory nerve cells and can periodically reactivate.

Maintaining normal immune function is therefore one important part of supporting the body’s natural defenses.

But zinc has another interesting characteristic.

Researchers have also investigated whether zinc can interact more directly with HSV itself.

How Could Zinc Interact With HSV?

Zinc has been studied for antiviral activity for several decades.

This is important because HSV must successfully attach to and enter a host cell before it can establish infection and replicate.

Laboratory findings help explain potential antiviral mechanisms, while human research provides an opportunity to see how those effects may translate clinically.

What Do Human Studies Show?

One of the more established areas of zinc research involves topical zinc preparations applied directly to herpes lesions.

In a double-blind, randomized, placebo-controlled clinical trial, researchers evaluated a topical zinc oxide/glycine cream in 46 people with facial or circumoral herpes infections.³

Participants who began applying the zinc cream within 24 hours of developing symptoms experienced cold sores for an average of approximately:

5 days with zinc oxide/glycine

compared with:

6.5 days with placebo.³

The zinc group also experienced improvements in several symptoms, including:

  • Blistering
  • Soreness
  • Itching
  • Tingling³

The timing may be particularly relevant.

HSV begins replicating before a cold sore is fully developed, so interventions that influence viral activity may have their greatest opportunity to work when used during the earliest stages of an outbreak.

Other clinical research has investigated topical zinc sulfate in people with recurrent genital herpes, with studies reporting reductions in recurrence with certain concentrations of topical zinc sulfate.⁴

Together, these findings provide human clinical evidence that specific topical zinc preparations may influence the course of recurrent HSV lesions.

What Does the Research Say About Oral Zinc and Cold Sores?

While much of the research on zinc and HSV has focused on laboratory models and topical preparations, there is also clinical evidence exploring oral zinc supplementation in people with recurrent cold sores.

A 2005 pilot study followed 20 people who experienced more than six episodes of recurrent herpes labialis per year. Participants took zinc sulfate providing 22.5 mg of zinc twice daily during selected months of the year and were followed for 12 months.⁵

During the study period, outbreaks decreased to fewer than four episodes per year, with an average of approximately three episodes. The average duration of each episode was about 5.7 days. The researchers concluded that systemic zinc sulfate appeared to reduce both the number of recurrent episodes and recovery time.⁵

These findings add an important dimension to the zinc-HSV story.

In addition to zinc’s established role in immune function and the clinical research on topical zinc, this pilot study suggests that systemic zinc supplementation may also have potential for people who experience frequent recurrent cold sores.

Because this was a small pilot study rather than a large randomized controlled trial, additional research would be valuable to determine which individuals are most likely to benefit and what dosing strategies are most appropriate.

Still, the findings provide promising human evidence that the potential role of zinc in recurrent herpes labialis may extend beyond topical use.

Why Zinc Status Still Matters

Zinc status may be particularly relevant for people who experience recurrent cold sores, both because of zinc’s fundamental role in immune function and because emerging human research has linked zinc status with the course of herpes labialis.

In a 2020 study comparing 43 people with recurrent herpes labialis with 42 people without a history of cold sores, researchers examined serum zinc concentrations and clinical characteristics.⁶

Among those with recurrent herpes labialis, researchers found a significant relationship between serum zinc levels and recovery time. **Lower serum zinc levels were associated with longer lesion recovery periods.**⁶

The study did not find a significant relationship between zinc levels and recurrence frequency, but the association with healing time adds to the evidence that zinc status may be clinically relevant in recurrent herpes labialis.

Combined with zinc’s established role in innate and adaptive immunity, these findings provide another reason to prioritize adequate zinc status as part of overall immune support.

  • Oysters and other shellfish
  • Meat
  • Poultry
  • Dairy products
  • Beans and lentils
  • Nuts and seeds
  • Whole grains

People who consume little or no animal products, have certain gastrointestinal conditions, or have difficulty absorbing nutrients may be at greater risk of inadequate zinc intake.

For these individuals, ensuring adequate zinc intake may be particularly important for maintaining normal immune function.

Can You Take Too Much Zinc?

Yes.

Zinc is an essential nutrient, but excessive supplementation can cause adverse effects.

High zinc intake can cause nausea, gastrointestinal discomfort, and other symptoms. Long-term excessive intake can also interfere with copper absorption and potentially lead to copper deficiency.

For this reason, zinc supplementation should focus on meeting nutritional needs and using evidence-informed amounts rather than assuming that increasingly large doses will necessarily produce stronger antiviral effects.

The available research suggests that zinc’s relationship with HSV may depend on several factors, including zinc status, dose, formulation, and route of administration.

Where Does Zinc Fit Into HSV Support?

HSV is a complex virus, and zinc appears particularly interesting because research has explored its potential role from several different angles.

Zinc supports normal immune function and antiviral immunity throughout the body.¹ Laboratory studies suggest that zinc ions can also interact directly with HSV and interfere with processes involved in viral attachment, penetration, and replication.¹,²

Importantly, the research extends beyond laboratory findings.

Topical zinc preparations have demonstrated benefits in human HSV studies, including improvements in outbreak duration, symptoms, and recurrence.³,⁴

A pilot study of systemic zinc supplementation in people with frequent recurrent cold sores also reported reductions in both outbreak frequency and recovery time.⁵

Observational research provides another piece of the picture: lower serum zinc levels have been associated with longer healing times in people with recurrent herpes labialis.⁶
