If you’ve ever searched for supplements for cold sores or herpes simplex virus (HSV), you’ve probably encountered the same recommendation:

Take L-lysine.

L-lysine has been associated with HSV support for decades and remains one of the most recognizable nutritional ingredients in the conversation around recurrent cold sores.

But there’s an important detail that often gets lost.

The amount of lysine you take may matter just as much as the fact that you’re taking it.

Clinical trials of L-lysine have used very different daily doses, which may help explain why research results have been inconsistent. A review of the available evidence found little evidence of benefit from doses below 1 gram per day, while doses above 3 grams per day appeared more promising.¹

That creates an interesting practical question.

If some of the more encouraging research involves several grams of lysine every day, is simply taking a small amount of lysine enough?

Let’s look at what the research actually says.

Why Is L-Lysine Studied for HSV?

L-lysine is an essential amino acid, meaning the body cannot produce it on its own and must obtain it through food.

Its relationship with HSV largely revolves around another amino acid: arginine.

HSV requires arginine during viral replication. Lysine and arginine have long been proposed to interact competitively within the body, leading researchers to investigate whether increasing lysine availability could influence conditions that favor HSV replication.¹

This biological hypothesis led to multiple clinical trials evaluating L-lysine supplementation for people experiencing recurrent herpes infections.

The results haven’t all been consistent.

But when researchers look more closely at the studies, one variable becomes particularly interesting:

dose.

With L-Lysine, Dose May Matter

One of the most useful reviews of L-lysine and HSV examined clinical studies conducted over several decades.

The authors found an important pattern.

Doses below 1 gram per day generally did not demonstrate convincing benefits for preventing or treating HSV lesions when they weren’t accompanied by a low-arginine diet.¹

At higher doses, however, the picture looked different.

The review concluded that doses above 3 grams per day appeared to improve participants’ subjective experience of HSV, although the authors emphasized that additional controlled studies are needed to determine the minimum reliably effective dose.¹

That distinction may help explain why people can have very different experiences with lysine.

Two supplements can both contain “L-lysine” on the label while delivering dramatically different amounts.

And based on the existing research, those differences may matter.

What Happened at 3 Grams Per Day?

One of the better-known studies of lysine for recurrent HSV was a double-blind, placebo-controlled, multicenter trial published in Dermatologica.

Participants in the treatment group took:

1,000 mg of L-lysine, three times per day, for six months.²

That’s a total daily dose of:

3,000 mg—or 3 grams—of L-lysine per day.

Among participants who completed the study, the lysine group experienced an average of 2.4 fewer HSV infections than the placebo group over the study period. Researchers also reported statistically significant improvements in symptom severity and healing time.²

These findings don’t mean that 3 grams is a guaranteed effective dose for everyone.

Other lysine studies have produced mixed results, and systematic reviews have highlighted limitations in the overall evidence base.¹,⁴

But the study illustrates an important point:

Some of the positive clinical evidence for lysine comes from doses substantially higher than the amounts people may assume they need when they see “L-lysine” on a supplement label.

What About Lower Doses?

Lower-dose research helps put that finding into perspective.

A placebo-controlled trial examining 1 gram of lysine per day did not demonstrate a significant reduction in recurrent herpes labialis when evaluated in a later systematic review.⁴

The broader review of lysine research similarly concluded that supplementation at doses below 1 gram per day appeared ineffective for HSV prophylaxis or treatment when not combined with a low-arginine diet.¹

This doesn’t establish a perfect cutoff where 999 mg “doesn’t work” and 3,000 mg suddenly does.

Nutrition research rarely works that way.

Instead, it suggests a dose-response issue worth paying attention to:

Lower doses have generally produced less convincing results, while some of the more positive clinical findings have occurred at substantially higher daily intakes.¹,²

The Practical Problem With High-Dose Lysine

Here’s where the research becomes relevant to everyday supplementation.

Three grams is a lot of lysine.

To reach 3,000 mg per day, someone using a product containing 500 mg of lysine per capsule, for example, would need six capsules per day just to reach the lysine dose used in the 1987 clinical trial.

And in that study, participants maintained the 3-gram daily intake for six months.²

That creates a practical challenge.

A strategy may look straightforward on paper, but if it requires several capsules every single day for months, adherence becomes an important part of the conversation.

This raises another question:

Does supporting HSV have to mean relying on a mega-dose of one ingredient alone?

Looking Beyond Lysine Alone

Lysine has the advantage of decades of research behind it, but HSV biology involves more than one pathway.

That’s one reason researchers have also explored formulations that combine lysine with other ingredients rather than using lysine in isolation.

One small pilot study evaluated a topical formulation containing L-lysine, zinc, and botanical ingredients in 30 adults with facial and circumoral herpes.³

The botanical ingredients included extracts traditionally used in topical cold sore products, alongside zinc and lysine.

Researchers monitored symptoms including:

  • Tingling
  • Itching
  • Burning
  • Tenderness
  • Swelling
  • Blistering
  • Soreness
  • Crusting³

By day three, 40% of participants had experienced complete resolution according to the study’s criteria, increasing to 87% by day six. No adverse events were reported.³

Those results are interesting, but there is an important limitation:

This was a small, open-label pilot study without a placebo control group.

It also evaluated the ingredients as a combination, meaning the study cannot tell us how much of the observed effect came from lysine, zinc, an individual botanical, or the formulation as a whole.³

So this research should not be interpreted as proof that combining ingredients is superior to lysine alone.

What it does show is that researchers have begun exploring a different question:

Rather than continuing to increase the dose of a single ingredient, can lysine be studied as part of a broader, complementary formulation?

Why Complementary Ingredients Are Interesting

Different nutritional and botanical ingredients may be studied for different reasons.

Lysine has primarily been investigated because of its proposed relationship with arginine and HSV replication.¹

Zinc, meanwhile, has its own body of research related to normal immune function and antiviral biology.

Botanical extracts may contain additional bioactive compounds that operate through still other mechanisms.

That doesn’t mean simply combining ingredients automatically makes a product more effective.

Formulation still needs to be evidence-informed, and each ingredient needs a clear rationale for being there.

But it does provide an alternative way of thinking about HSV support.

Instead of asking:

“How much lysine can we fit into a supplement?”

A more useful question may be:

“How can lysine be used alongside complementary ingredients that approach HSV and immune support through different biological pathways?”

Where Does This Leave L-Lysine?

L-lysine shouldn’t be dismissed.

It’s one of the most researched nutritional ingredients associated with recurrent HSV, and a controlled clinical trial using 3 grams per day reported reductions in recurrence along with improvements in symptoms and healing time.²

At the same time, the evidence isn’t strong enough to say that lysine prevents or treats HSV for everyone.

Reviews of the literature continue to describe inconsistent findings, particularly at lower doses, and more high-quality research is needed.¹,⁴

The most interesting takeaway may therefore be less about whether lysine “works” or “doesn’t work.”

It’s that dose matters.

And once the doses used in positive studies are considered, so does practicality.

The Bottom Line

L-lysine has earned its reputation as one of the best-known nutritional ingredients for HSV, but simply seeing lysine on a supplement label doesn’t tell the whole story.

Research suggests that dose may be a major part of the equation.

A review of the available evidence found little support for doses below 1 gram per day without dietary arginine restriction, while doses above 3 grams per day have produced more promising findings.¹

In one double-blind, placebo-controlled trial, participants took 3 grams of lysine every day for six months and experienced fewer HSV infections along with improvements in symptom severity and healing time.²

But consistently reaching that amount can also mean taking multiple capsules or tablets every day.

That’s why another area of research is particularly interesting: using lysine alongside complementary ingredients such as zinc and botanical extracts, rather than assuming that the only way to formulate around lysine is to rely on increasingly large doses of a single amino acid.³

The evidence for these combinations is still preliminary, and they should not be considered replacements for antiviral medications or evidence-based medical care.
