When we think about the microbiome, we usually think about the gut. But your digestive tract isn’t the only place where beneficial bacteria play an important role.
Your vagina also has its own unique microbiome, and emerging research suggests these two microbial communities may be more connected than we once thought. Scientists refer to this relationship as the gut-vagina axis.¹
While research is still developing, evidence suggests that gut health may influence vaginal health through the immune system, hormones, and the metabolites produced by beneficial bacteria.¹,² Here’s what you need to know.
What Is the Gut-Vagina Axis?
The gut-vagina axis describes the communication between the gut microbiome and the vaginal microbiome through several interconnected systems, including:
- The immune system
- Hormonal regulation (particularly estrogen)
- Microbial metabolites
- Inflammatory pathways¹,²
Unlike the gut, where greater bacterial diversity is generally considered beneficial, a healthy vaginal microbiome is typically less diverse and dominated by Lactobacillus species. These beneficial bacteria produce lactic acid, helping maintain an acidic vaginal pH that discourages the growth of harmful microorganisms.⁴
Researchers believe changes in the gut microbiome may influence this delicate balance, although many of these relationships are still being investigated.¹
Why Does the Vaginal Microbiome Matter?
A healthy vaginal microbiome does more than prevent infections.
It also helps:
- Maintain an acidic vaginal pH
- Support the vaginal barrier
- Regulate local immune responses
- Reduce the growth of potentially harmful bacteria and yeast⁴
When this balance is disrupted, the risk of certain conditions may increase, including:
- Bacterial vaginosis (BV)
- Yeast infections
- Urinary tract infections (UTIs)
- Persistent HPV infection in some individuals²,⁴
It’s important to note that vaginal health is influenced by many factors, including hormones, sexual activity, medications, hygiene practices, and genetics; not just the gut microbiome.²
How Could Your Gut Influence Your Vaginal Health?
Researchers have proposed several ways these two microbiomes may interact.
Immune Function
Around 70% of the body’s immune cells are associated with the gut. A healthy gut microbiome helps regulate immune responses throughout the body, including at mucosal surfaces such as the vagina.⁵
Disruptions in gut bacteria may contribute to altered immune signaling, which could affect vaginal microbial balance.
Hormone Regulation
The gut microbiome also influences how estrogen is metabolized through a collection of bacteria sometimes called the estrobolome. Healthy estrogen levels help maintain glycogen within vaginal tissues, providing fuel for beneficial Lactobacillus bacteria.²
Changes in hormone levels during menopause, after antibiotics, or throughout the menstrual cycle can also affect the vaginal microbiome.⁴
Shared Microbial Communities
Some bacterial species found in the vagina may originate from the gastrointestinal tract, and researchers continue to study how microbes move between these ecosystems.¹
Although this connection appears biologically plausible, experts agree that much more research is needed to understand exactly how these microbial communities influence one another.
Can Improving Gut Health Improve Vaginal Health?

Research on oral probiotics for vaginal health is promising but remains mixed, with benefits appearing to depend on the specific bacterial strains studied rather than probiotics in general.²
